Records are resolved by designation. No further action is required.
Standard forms
- F-01 Volume Assessment Report Declared against measured. Filed whether or not the two agree.
- F-02 Occupancy Incident Record An occupancy which changed without a personnel movement to account for it.
- F-03 Witness Validation Record Confirms a witness held no assignment in what they witnessed.
- F-04 Retained Movement Record Designation, origin operator, duration, and release condition.
- F-05 Movement Release Request Submitted to release retained movement. Refusal is recorded, not appealed.
- F-06 Route Continuity Report Arrival, departure, and whether the route completed.
- F-18-B Duplicate Operator Report Reporting operator and observed operator are recorded separately, and may be the same designation.
- F-07 Assignment Review Responsibility does not establish identity. Reviewed on that basis.
- F-08 Public Exposure Request Request that a volume be represented on the public plan.
- F-09 Withdrawal Review Withdrawal of public representation. The volume remains registered.
- F-10 External Contact Record What was exchanged on Route 00-EXT, and with whom, where established.
- F-11 Closure Exception Record Filed where a closure could not be resolved by the assessor.
Reproduction
These are the standard forms. Local departments may maintain additional forms; a local form does not override the manual without a recorded revision.
Forms carry the metadata they were filed with. Where that metadata disagrees with the current register — and in several places it does — the form is not corrected, because a corrected form is a different form.
Nothing in this archive is a government record, a third-party certificate, or a licence. These are the company’s papers about its own work.
Public archive
Documents prepared for public access are held by Public Revision. Annual reports. Vacancy lists. Structural bulletins. Telephone lists. Every document bears the same revision date: 17 November 1997.
